Joe Fontenot MD, CLVT Dr. Fontenot is a medical doctor, Certified Low Vision Therapist and Medical Director of Community Services for Vision Rehabilitation. He has low vision himself, and understands the problems and needs of those with vision impairment.

Stephanie Montgomery, OD.

Stephanie Montgomery
Dr. Montgomery is an Optometrist specializing in Low Vision Rehabilitation. She evaluates all patients, provides individualized care and rehabilitation. She prescribe specialized glasses, optical aids and devices for those with vision loss.

Carol Evans, OTR/L

Carol Evans is our Occupational Therapist. Acting on the recommendations of Dr. Fontenot, she helps each patient achieve the highest possible level of function. With more than 20 years of experience, Carol is experienced in all aspects of training and education of the blind and visually impaired. She is a certified Braille and Orientation and Mobility Instructor. She makes home visits as well as seeing patients at our office.

Kim Bufalo, R.N., B.A.

Office Manager Kim Bufalo is a Registered Nurse, with extensive experience in the health care field and computers. She comes to us from Computer Products and Services Incorporated (CPSI), a company specializing in medical computer software. She has experience working with the visually impaired in hospital and clinic settings and has Optometric office experience. She brings knowledge and enthusiasm to her position as Office Manager.

Betty Wainwright, LPN

Betty Wainwright, a Licensed Practical Nurse, is our receptionist and office facilitator. She is the initial contact for our office. With considerable experience in many aspects of health care, she is able to provide information and help to the visually impaired.

Joanne Leon

Joanne Leon performs many tasks at CSVR. She acts as receptionist, schedules appointments and arranges insurance pre-certifications and helps with billing. With many years of experience working in the medical field, she is an invaluable asset to CSVR.

Karla Mitchell

Karla Mitchell is our medical insurance and reimbursement specialist. .For those patients with insurance coverage, she helps guide them through the often complicated paperwork involved.

Marcie Ortega is our Montgomery office manager. With experience as a former Optometric office manager and with enthusiasm and energy she has started our Montgomery program.
